Background Checks:

All selected coaches are required to complete a background check prior to receiving rosters and equipment.  Once notified by the administrator that you have been selected to coach, please follow the link below to complete your background check.

Concussion Training:

All coaches are required to complete a concussion training course provided by the Center for Disease Control and Prevention.  This is a 30 minute training course in which the recipient will receive a certificate upon completion.  The certificate will then need to be emailed to the head administrator or the child safety coordinator.  Please follow the link below for the training course.

Concussion Policy:

If a coach believes a player is suffering from a concussion, the coach must remove the player from the practice/game.  Once removed, the coach must email the Child Safety officer ([email protected]) with the child's name.  The child may not return to practice or game play until the child's doctor provides a written slip clearing the player.

Waiver / Exclusion Clause

I, the parent/guardian/participant, in consideration of being allowed to participate in any way in the NORTHERN KENTUCKY YOUTH ASSOCIATION (hereby known as N.K.Y.A.) sports/activities program and related events and using the facilities provided by N.K.Y.A. does so at his/her/my own risk. N.K.Y.A., its board of directors, volunteers, agents, shall not be liable for any damages whatsoever arising from any personal injury or property loss sustained by participant and his/her/my family in or about any programs provided by the N.K.Y.A. I acknowledge that I am aware of the risks inherent in participating in the N.K.Y.A. programs/activities and understand that they may require physical exertion; and could potentially lead to injuries that may cause permanent disability and/or death. Participants and parents assume full responsibility for all injuries and damages which may occur in or about any programs and on any premises that N.K.Y.A. uses. He/she/I do or does hereby fully and forever release, discharge, and hold harmless the N.K.Y.A., all associated facilities and its owners, employees and agents from any and all claims, demands, damages, right of action, present or future resulting from or arising out of any person's participation in any programs/activities. Also, waiver all rights to any photos taken for use in any Northern Kentucky Youth Association publication. $1 of your registration fee is applied to membership dues.

 

 

Concussion Waiver

I have read the Northern Kentucky Youth Association Concussion Information Sheet and understand what a concussion is and how it may be caused. I also understand the common signs, symptoms, and behaviors. I agree that my child must be removed from practice/play if a concussion is suspected. I understand that it is my responsibility to seek medical treatment if a suspected concussion is reported to me. I understand that my child cannot return to practice/play until providing written clearance from an appropriate health care provider to his/her coach. I understand the possible consequences of my child returning to practice/play too soon. COVID-19

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. It is believed that an individual can be infected with COVID-19 without their knowledge and be asymptomatic. Northern Kentucky Youth Association has put in place preventative measures to reduce the spread of COVID-19. However, Northern Kentucky Youth Association cannot guarantee that I, the parent/guardian/participant, or anyone else will not become infected with COVID-19, including my spouse, children, guests, unborn child, or relatives. Participation in a Northern Kentucky Youth Association athletic sports program(s), related event, or activity, could increase the risk of contracting COVID-19. By signing this agreement, I ACKNOWLEDGE the contagious nature of COVID-19 and VOLUNTARILY ASSUME THE RISK that I and my children may be exposed to or infected by COVID-19 by participating in a Northern Kentucky Youth Association athletic sports program(s), related event, or activity, and that such exposure or infection may result in personal injury, illness, permanent disability, and death to myself, my spouse, children, guests, unborn child, or relatives. I understand that the risk of becoming exposed to or infected by COVID-19 at a Northern Kentucky Youth Association athletic sports program(s), related event, or activity may result from the actions, omissions, or negligence of myself or others, including, but not limited to, Northern Kentucky Youth Association volunteers, and program participants. I UNDERSTAND AND VOLUNTARILY ACCEPT AND ASSUME ALL the foregoing risks related to COVID-19 and accept sole responsibility for any injury or illness that may occur. Further, I UNDERSTAND AND AGREE that this release includes any Claims based on the actions, omissions, or negligence of Northern Kentucky Youth Association, its volunteers, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Northern Kentucky Youth Association athletic sports program(s), related event, or activity.
